Community Service Volunteer Contact Form

  1. Court-ordered community service volunteers must follow these steps to volunteer:

    1. Fill out this Community Service Volunteer Contact Form below.
    2. Print and fill out the required documents: Printable Application, Community Service Volunteer Hours Form, and Community Service Volunteer Rules.
    3. Contact the Community Service Volunteer Coordinator at 622-0525, ext 221 to set up a time to bring all completed forms and required court documents to the Community Food Bank Receptionist. The receptionist is located on the first floor of the main facility at 3003 S. Country Club Road and can be reached between 7:30 AM and 4:30 PM.

  7. Charges: We do not accept volunteers who have been charged with violent offenses such as but not limited to: domestic violence, assault, sexual harassment, sexual abuse, disorderly conduct, armed robbery or aggravated assault.

    I hereby agree to hold harmless and waive any and all claims or causes of action against the COMMUNITY FOOD BANK arising out of any cause whatsoever, including but not limited to claims arising out of the negligence or intentional conduct of its employees or agents. I attest that I have not engaged in any crimes involved with the use of a weapon or crimes of violence, including charges of domestic violence, assault, sexual harassment, sexual abuse, disorderly conduct, armed robbery, or aggravated assault. I attest that I am physically fit and prepared to perform the tasks assigned to me as a CFB volunteer subject to all personal limitations and restrictions. I further agree to use my personal insurance as the primary provider in the event of injury due to my work as a volunteer for the COMMUNITY FOOD BANK. I also grant the COMMUNITY FOOD BANK full permission to use photographs of me.
